In short
Biological fillers, or biostimulators, are injectables that prompt your own body to produce collagen rather than adding volume directly. Sculptra is poly-L-lactic acid and works almost entirely by stimulation, so the result builds over three to six months across a series. Radiesse is calcium hydroxylapatite and does both: it adds volume on the day and stimulates collagen afterwards. Both are $900 a syringe at BHRC and neither can be dissolved.
A hyaluronic acid filler is exactly what it sounds like: a gel that occupies space, so you see the result the moment it is placed and it can be dissolved if you hate it. A biostimulator does something different and slower. Sculptra irritates the tissue just enough that your body responds by laying down collagen around the particles, over weeks and months. You leave looking essentially unchanged, and three months later people say you look well without knowing why. That is the appeal, and the cost of it is that there is no undo button.

Sculptra is usually a series of three vials spaced four to six weeks apart, because collagen takes time and you cannot judge the result between sessions. Radiesse is more often a single syringe per area.

How it works
Two different molecules, two different timelines.
Sculptra is poly-L-lactic acid, a material used in dissolvable sutures for decades. Injected into the deep dermis it triggers a controlled response that lays down new collagen around each particle as the particle itself is absorbed.
Radiesse is calcium hydroxylapatite microspheres in a gel carrier. The gel adds volume immediately, and as it is absorbed the microspheres provide a scaffold that new collagen forms along.
Both are building tissue rather than occupying space, which is why the result is judged at three months rather than two weeks, and why neither can be dissolved the way hyaluronic acid can.

Collagen builds over months, so the change is invisible week to week and obvious in photographs. Nobody catches you having had something done.
Sculptra suits diffuse volume loss, which is what most ageing actually is, rather than a single fold.
Areas where hyaluronic acid can look obvious and a biostimulator generally does not.
Radiesse holds its position well, which is why it is often chosen along the jaw border.
One of very few products with an established use there, and hands give age away long before faces do.
New collagen thickens the skin itself, which is a different effect from filling underneath it.
Hyaluronic acid can be reversed with hyaluronidase. Sculptra and Radiesse cannot. That is the single most important difference and it is why we usually start people on HA.
Aftercare
Including the massage nobody remembers to do.
Swelling and possible bruising at injection points. What you see now is the water in the product, not the result.
Five minutes, five times a day, for five days. It matters: it distributes the particles evenly and reduces the risk of nodules. This is the most commonly skipped instruction in aesthetics.

Biological fillers are injectable treatments made from biocompatible materials (most commonly hyaluronic acid) that restore volume, smooth wrinkles, and enhance facial contours. Once injected, the filler integrates with your tissue and attracts water molecules, creating natural-looking fullness. They work by replacing volume lost due to aging and stimulating your skin’s own collagen production.
Filler longevity depends on the type of filler used and the treatment area. Lip fillers typically last 6-12 months, while cheek and jawline fillers can last 12-24 months. Areas with more movement (like lips) metabolize filler faster. Your provider will discuss expected duration based on your specific treatment plan during your consultation. Are dermal fillers safe?
FDA-approved dermal fillers have an excellent safety profile when administered by trained professionals. Common side effects include temporary swelling, redness, bruising, and tenderness at injection sites—these typically resolve within 1-2 weeks. Serious complications are rare but can include infection or vascular occlusion, which is why choosing an experienced injector is crucial.
Most patients describe filler injections as mild discomfort rather than pain. We apply topical numbing cream before treatment, and most fillers contain lidocaine to minimize sensation during injection. Using cannulas (blunt-tipped needles) in certain areas also reduces discomfort. Most patients are pleasantly surprised by how comfortable the experience is.
Yes! Hyaluronic acid fillers can be dissolved with an enzyme called hyaluronidase if you’re unhappy with your results or experience complications. This is one of the major advantages of HA fillers—they’re completely reversible. The dissolving process works within 24-48 hours, returning the treated area to its pre-treatment state.
Botox and fillers address different concerns. Botox relaxes muscles to smooth dynamic wrinkles (those caused by movement, like forehead lines and crow’s feet). Fillers add volume to smooth static wrinkles, restore lost volume, and enhance facial contours. Many patients combine both treatments for comprehensive facial rejuvenation—Botox for the upper face and fillers for the mid and lower face.
The amount of filler needed varies based on your goals and facial anatomy. Lip enhancement typically requires 0.5-1 syringe, while cheek augmentation may need 1-2 syringes per side. Full-face rejuvenation often requires 3-6 syringes. During your consultation, your provider will assess your face and recommend the appropriate amount to achieve natural-looking results.
Before treatment, avoid blood thinners, alcohol, and supplements like fish oil, vitamin E, and ginkgo biloba for 1 week to minimize bruising. After treatment, avoid strenuous exercise, alcohol, and excessive heat (saunas, hot yoga) for 24-48 hours. Don’t massage the treated areas unless instructed, and sleep elevated the first night to reduce swelling. Individual results may vary.
