In short
Under-eye treatment addresses three separate problems that look alike. Hollowing is lost volume in the tear trough and responds to filler. Dark circles are often pigment or visible vasculature through thin skin, which filler does not fix. Crepey, lax skin is a skin-quality problem treated with PRP or radiofrequency. Working out which you actually have is most of the consultation.
Almost everyone who books an under-eye consultation says the same sentence: I look tired. What is actually causing it varies completely. If the orbital rim has hollowed, there is a shadow and filler genuinely helps. If the skin is thin enough that the underlying muscle and vessels show through, that is a colour problem and adding gel underneath it will not change the colour. If the skin itself is crepey, filler makes it worse by stretching it. Same complaint, three different treatments, and getting it wrong here is more visible than anywhere else on the face.

How it works
Matching the treatment to the actual cause.
For true hollowing, a small amount of hyaluronic acid is placed deep, on the bone beneath the tear trough, rather than superficially under the skin. Depth is what separates a natural result from a visible one.
For skin quality, PRP or low-energy radiofrequency microneedling thickens and improves the skin itself over a series, which is the only thing that helps when thin skin is letting colour show through.
For pigment, neither works. Topical treatment, sun protection and occasionally a peel are what address pigmented circles, and you will be told that rather than sold a syringe.

Starts working: n/a. Lasts: n/a. If the cause is pigment, or true puffiness from fat prolapse, no injectable here will help and some will make it worse. That is a real consultation outcome and it is one we give.

Restoring lost volume along the orbital rim removes the shadow that reads as exhaustion.
PRP and radiofrequency thicken thin lower-lid skin, which is the only real answer when vasculature shows through it.
Texture improves with treatments that work on the skin rather than underneath it.
Under-eye filler can migrate or hold water, producing puffiness weeks or months later. It happens when too much is placed, or placed too superficially, or when a product too hydrophilic for the area is used. It is why we use less product, place it deeper, and choose Restylane or Belotero rather than a thicker gel.
Hyaluronidase dissolves hyaluronic acid filler, including migrated or over-placed filler from another clinic. A large share of our under-eye consultations are exactly this.
Pigmented circles and true fat prolapse do not respond to filler. In the second case the honest answer is surgical.
Thin skin, poor lymphatic drainage and proximity to vessels that must be avoided. This is not a treatment to have from whoever is cheapest.
Aftercare
This area swells and bruises more visibly than any other.
Swelling and bruising are common and highly visible here. Ice gently, sleep elevated, and expect to look worse before better.
Bruising resolves. Swelling settles more slowly than elsewhere because lymphatic drainage in this area is poor.
The true result appears. Judging any earlier than this is pointless and causes unnecessary panic.
Come back. Delayed swelling weeks or months on is usually the product holding water and is straightforwardly correctable.
Salt, alcohol and strenuous exercise for 24 to 48 hours. All three make under-eye swelling worse.
Four weeks minimum. This is not an area to treat close to a photograph.
12 to 18 months for filler. Most patients need less at the second appointment than the first.
Aftercare

Under eye concerns have multiple causes including genetics (thin skin, visible blood vessels), aging (volume loss, fat pad descent, collagen depletion), lifestyle factors (poor sleep, allergies, dehydration), and bone structure. During your consultation, we’ll identify the specific causes affecting you to recommend the most effective treatment approach.
Under eye filler results typically last 12-18 months, though this varies by individual. The under eye area has limited movement compared to other facial areas, which helps fillers last longer. Many patients find they need less filler with each subsequent treatment as the area maintains better volume over time. Touch-up treatments can extend your results. Is under eye filler safe?
When performed by experienced injectors like our team at BHRC, under eye filler is very safe. Common side effects include temporary swelling, bruising, and tenderness that resolve within 1-2 weeks. Rare risks include asymmetry (easily correctable), Tyndall effect (bluish tint if placed too superficially), and vascular complications. Our providers use advanced techniques including cannulas to minimize risks.
Both are excellent hyaluronic acid fillers for the under eye area. Belotero has a thinner consistency and integrates very smoothly into tissue, making it ideal for superficial placement and thin skin. Restylane offers slightly more structure for deeper volume loss. Your provider will recommend the best option based on your anatomy and the depth of your hollows.
Morpheus8 uses radiofrequency energy delivered through microneedles to stimulate collagen production and tighten skin. For under eyes, it improves crepey skin texture, reduces fine lines, and provides subtle tightening of loose skin. It’s often combined with filler—Morpheus8 addresses skin quality while filler addresses volume loss.
PRP (Platelet-Rich Plasma) is derived from your own blood. We draw a small amount, spin it to concentrate the platelets, then inject or microneedle it into the under eye area. The growth factors stimulate collagen production, improve skin quality, and can help reduce dark circles by improving blood vessel health. Results develop gradually over 4-6 weeks.
Filler: Minimal downtime. Expect swelling for 3-7 days and possible bruising for 1-2 weeks. Most patients return to work immediately. Morpheus8: Redness and swelling for 3-5 days, with full recovery in 1 week. PRP: Minimal downtime, possible bruising for a few days. We recommend avoiding strenuous exercise for 24-48 hours after any treatment.
For many patients, yes! Non-surgical treatments are ideal for volume loss, dark circles, and mild skin laxity. However, if you have significant fat pads causing puffy bags, or severe excess skin, surgery may provide better results.
