Kybella vs CoolSculpting for a Double Chin: How to Choose

Injectables · Kybella · Comparison

Kybella and CoolSculpting both destroy fat under the chin. One is a series of injections, the other a cooling applicator. Here is how they compare on the things that decide it, using the manufacturers’ own labels.

The short answer. Neither is better for everyone. Kybella is an FDA-approved injectable that can follow the shape of a small or irregular pocket, usually over two to four or more treatments, with swelling in 87% of trial patients. CoolSculpting is an FDA-cleared cooling device for fat under the chin and jawline, with no needles. Neither tightens loose skin. Six BHRC clinics offer both, so your provider can recommend either.

Two ways to destroy the same fat

A double chin is usually a pocket of fat under the chin, sometimes with loose skin over it. Kybella and CoolSculpting both destroy fat cells in that pocket; they differ in how they get there.

  • Kybella is an injectable drug, deoxycholic acid, FDA approved in 2015 to improve moderate to severe fullness from fat under the chin in adults. Injected into the fat, it breaks down the fat cell membranes it reaches.
  • CoolSculpting and CoolSculpting Elite are devices. An applicator on the skin cools the fat, a process called cryolipolysis, and they are FDA cleared for visible fat bulges under the chin and under the jawline. The manufacturer says treated fat is eliminated permanently and that it is not a weight loss treatment.

Neither one tightens loose skin. If the problem is skin rather than fat, a skin treatment such as Morpheus8 or Ultherapy is usually the better starting point, and our double chin guide walks through that choice.

Which is better for a double chin?

Neither is better for everyone, and a clinic that sells only one of them will tend to tell you it is the one. The questions that decide it:

How much fat, and what shape?

An applicator has to fit the area it treats. Injections can follow the outline of a pocket that is small or irregular. Your provider pinches the area to judge the thickness and shape of the fat before suggesting either.

How do you feel about needles and swelling?

Kybella means injections and, for most people, real swelling: 87% of trial patients had it, and in 20% it lasted more than a month. CoolSculpting has no needles, though it has its own temporary swelling, tenderness and numbness. If needles are a dealbreaker, that settles it.

Is loose skin part of the picture?

Kybella’s label asks providers to weigh excessive skin laxity before treating, because removing fat can make loose skin more visible. CoolSculpting is also FDA cleared to affect the appearance of lax tissue with chin treatments. Either way, significant loose skin is usually a skin-tightening conversation.

Any medical reasons to rule one out?

Swallowing problems or an infection in the area point away from Kybella. The cold-related blood conditions listed above rule out CoolSculpting. Bleeding problems and blood thinners matter for both, because bruising is common.

Can you combine them?

Some people end up with one for the main pocket and the other for what is left, or a fat treatment followed by a skin treatment. The manufacturers do not publish a combined protocol, so this is a plan your provider builds with you, not a package.

Where BHRC offers both

These six clinics list Kybella and CoolSculpting, so the recommendation is not shaped by which one a clinic happens to have. West Hollywood, Valencia and Westlake Village offer Kybella only.

Clinic Kybella starting price CoolSculpting list price
West Los Angeles, Los Angeles $800 CoolSculpting Elite, $800 a cycle
Downtown Summerlin, Las Vegas $800 CoolSculpting Elite or Legacy, $800 a cycle

List prices, November 2026, before any promotion. Henderson lists Kybella per vial; the other clinics list a starting price per treatment, and your provider confirms the vials per treatment and the number of treatments. Frequently asked questions

Is Kybella or CoolSculpting better for a double chin?

Neither is better for everyone. Kybella is injected and can follow the shape of a small or irregular pocket; CoolSculpting uses an applicator with no needles. The size and shape of the fat, how you feel about needles and swelling, and whether loose skin is involved decide it.

Is Kybella or CoolSculpting more permanent?

Both destroy the fat cells they treat. Allergan says further Kybella treatment is not expected once you reach your goal, and a three-year follow-up found the improvement maintained in most patients. The CoolSculpting manufacturer says treated fat is eliminated permanently. Remaining fat cells can still grow with weight gain.

Which has more downtime, Kybella or CoolSculpting?

Swelling is the main thing to plan around with Kybella: 87% of trial patients had it, and in 20% it lasted more than a month. CoolSculpting lists temporary redness, swelling, bruising and tenderness, with numbness or tingling that can last several weeks. Your provider can tell you what to expect for your chin.

Does CoolSculpting work under the chin?

Yes. CoolSculpting and CoolSculpting Elite are FDA cleared for visible fat bulges under the chin and under the jawline.

Can Kybella be used on the jawline?

No. Kybella is approved only for fat under the chin, and its label reports serious reactions after use elsewhere. CoolSculpting is cleared for under the jawline.

Which BHRC clinics offer both?

West LA, Henderson, Downtown Summerlin in Las Vegas, Paradise Valley near Scottsdale, Houston CityCentre and Houston River Oaks.
