Kybella Swelling and Results: What to Expect, Treatment by Treatment

Injectables · Kybella · Recovery

Swelling after Kybella is normal, sometimes long, and the reason most people misjudge their first treatment. Here is what the FDA label says about how long each reaction lasts and when results actually appear.

The short answer. Swelling is the most common reaction, in 87% of trial patients. It is heaviest in the first days and settles over weeks, but lasted more than 30 days in 20%, and numbness lasted that long in 42%. Results build over a series: Allergan says many people see visible results after two to four treatments, spaced at least a month apart, and the trials measured results 12 weeks after the last one.

Why Kybella swells

Kybella is deoxycholic acid. Its prescribing information calls it a cytolytic drug: injected into the fat under the chin, it destroys the fat cell membrane. The body then clears those cells, and that clean-up is an inflammatory process. So the swelling is not a sign that something has gone wrong. It is how the treatment works, and the label lists it as the most common reaction.

How long Kybella swelling lasts

For most people swelling is heaviest in the first days after a treatment and settles over the following weeks. The label does not give a day-by-day schedule, and we would rather not invent one, but it does say which reactions lasted more than 30 days in more than 10% of patients:

  • Numbness: 42% of patients
  • Swelling: 20%
  • Pain: 16%
  • Firmness: 13%

In other words, about one person in five still has some swelling a month on. That is why treatments are spaced at least a month apart, and why we suggest booking away from weddings, photographs and anything where your jawline is the point.

Plan the calendar first. If you have an event, count back at least a month from it before your last planned treatment, and longer if you tend to swell or bruise.

The Kybella timeline, treatment by treatment

When What happens
Treatment day About 15 to 20 minutes of injections, according to Allergan. Ice and topical or injected numbing are used for comfort. Swelling, tenderness and numbness start the same day.
The following weeks Swelling and tenderness ease. Numbness often lasts longer than swelling. Fat cells are being cleared in the background, so the change is gradual.
At least 1 month later Your next treatment, if you need one. The label allows up to six, no less than a month apart. Later treatments usually use less product because there is less fat left.
After 2 to 4 treatments When Allergan says many people see visible results. In the trials, 59% of patients had all six.
12 weeks after the last treatment When the trials measured results: 66.5% to 70% of Kybella patients improved by at least one grade on both the clinician and patient ratings, against 18.6% to 22.2% on placebo.
Years later A follow-up of trial patients found the improvement maintained for three years in most of them.

When will I see Kybella results?

Not after the first visit, for most people. Allergan reports the share of patients with at least a one-grade improvement after each treatment:

After treatment 1 2 3 4 5 6
Kybella 8% 28% 43% 55% 66% 72%
Placebo 2% 7% 12% 14% 15% 22%

The pattern is steady: each treatment adds to the last. If you judge Kybella on how your chin looks a week after one treatment, you are mostly looking at swelling.

Are the results permanent?

Kybella destroys the fat cells it reaches, and Allergan says that once you reach your goal, further treatment is not expected. The three-year follow-up supports that for most patients. The fat cells that remain can still grow, so significant weight gain can change your profile again.

Side effects that are not just swelling

Two warnings on the label are worth knowing before you book:

  • An uneven smile. About 4% of trial patients had a temporary injury to the nerve that moves the corner of the mouth. Every case resolved without treatment, with a median of 44 days. Injectors avoid it by staying inside the marked area under the chin and below the jawline.
  • Difficulty swallowing. Reported in 2%, alongside swelling and pain, and resolved with a median of 3 days. People with current or past swallowing problems should not have Kybella.

The label also lists hair loss in the treated area, skin sores and tissue damage from injections that are too shallow, nodules, masses and infection. Contact your provider if you notice an uneven smile, facial weakness, trouble swallowing, open sores or drainage.

What helps with recovery

  • Cold packs on the area in the first days.
  • Tell your provider beforehand about blood thinners, aspirin and supplements that affect bleeding, because bruising is common. Never stop a prescribed blood thinner without the doctor who prescribed it.
  • Keep the next treatment at least a month away, and say so if you are still swollen when it comes round.

Frequently asked questions

How long does swelling last after Kybella?

It is usually heaviest in the first days and settles over the following weeks. In the trials, swelling lasted more than 30 days in 20% of patients.

How long does numbness last after Kybella?

Numbness under the chin often outlasts the swelling. It lasted more than 30 days in 42% of trial patients.

When do you see results from Kybella?

Gradually, treatment by treatment. Allergan says many people see visible results after two to four treatments; the trials measured results 12 weeks after the last one.

Is Kybella swelling worse after the first treatment?

The first treatment usually uses the most product, an average of 6.4 mL in the trials against 4.4 mL by the sixth, because there is more fat to treat.

What should I avoid after Kybella?

Plan around events and photographs. Use cold packs, and tell your provider about blood-thinning medicines and supplements before treatment, because bruising is common.

When should I call my provider after Kybella?

If you notice an uneven smile, facial weakness, trouble swallowing, open sores or drainage from the area.

Are Kybella results permanent?

Treated fat cells are destroyed, and a three-year follow-up found the improvement held in most patients. Remaining fat cells can still grow with weight gain.
